Abdomen swollen and deformed ? That's almost always a cancer of some kind. Cystic kidneys, or uterine cancers.  Again this complaint does require specialist attention with no guarantee, of course, that the koi will recover .. although there are many cases where surgery has helped the koi to continue to survive for quite a while following surgery.
